Perhaps from the Celtic-Irish mythology, Balor is the god of death and the king of the Fomorians, a race of demonic giants.
Dunno 'bout the sword tho... ;)

Using my spectacles of divination +2, and consulting my AD&D Monster Manual ('78 edition) I find that Type VI demons are unique creatures, one of which is named "Balor." The illustration looks to be based on Tolkien's balrog, because it carries a many-headed whip, has wings that appear to be flaming, and a lightning-shaped sword.
I think in the current iteration of the rules, a Balor is a type of demon, rather than a unique creature.
